摘要

目的:探讨针刺治疗糖尿病合并冠心病的临床疗效及取效机制。方法:采用多中心、随机对照、盲法原则

将120例糖尿病合并冠心病患者随机分为观察组和对照组各60例。在常规糖尿病治疗的基础上

观察组采用调理脾胃针法

取曲池、合谷、血海、足三里、阴陵泉、丰隆、地机、三阴交等穴针刺;对照组按《针灸学》取胃脘下俞、肺俞、脾俞、肾俞、阳陵泉等穴针刺。2组均每日治疗2次。以临床症状体征、心绞痛发作次数、持续时间、硝酸甘油服用量;血糖、血脂、尿白蛋白排泄率、尿β2微球蛋白、尿单核细胞趋化蛋白-1(MCP-1);心电图、心脏彩色多普勒等进行临床疗效评定。结果:调理脾胃针法不仅能改善患者的症状体征

而且能改善ST段下移的程度和左室舒张功能

对患者的糖、脂代谢、尿白蛋白水平亦有良性的调节作用

与对照组比较差异有统计学意义(P<0.01或P<0.05)。观察组心电图疗效的显效率为50.00%

优于对照组的13.79%。结论:调理脾胃针法可改善糖、脂异常造成的心血管损害

降低尿蛋白水平

抑制MCP-1过度表达

并能减轻心肌负荷

提高心搏出量。
