摘要

目的:从生物力学角度

探讨针刀松解治疗膝骨性关节炎的作用机制及方法。方法:将14例膝骨性关节炎患者膝关节周围92个痛点行针刀松解治疗

测量治疗前后局部软组织的力—位移曲线

得出500 g压力时该痛点的位移大小(L_(500g))

同时记录该点疼痛视觉模拟评分(VAS)。结果:痛点治疗前L_(500g)为(4.72±1.03)mm

治疗后为(5.39±1.01)mm

前后差异有非常显著性意义(P<0.01)

同时治疗前VAS评分为(7.10±1.49)分

治疗后为(1.49±1.24)分

前后差异也有非常显著性意义(P<0.01)

且L_(500g)的改变值与VAS的改变值二者有直线相关关系(P<0.01)。结论:膝骨性关节炎局部疼痛与该处软组织张力有密切联系

针刀松解可以通过松解局部挛缩、粘连的软组织

消除其对神经血管的压迫或牵拉

达到缓解张力性疼痛的目的

并最终恢复膝关节内部的力平衡。
