摘要

目的:探讨针刺治疗糖尿病肾病的临床疗效及作用机制。方法:采用多中心、随机对照、盲法原则

将130例糖尿病肾病患者分为观察组和对照组各65例。在常规糖尿病治疗的基础上

观察组采用调理脾胃针法

取曲池、支沟、合谷、血海、足三里、阴陵泉、丰隆、地机、三阴交、太冲、天枢、膏肓、肾俞、白环俞及中脘、中极穴针刺;对照组按《针灸学》取穴:肾俞、太溪、三阴交、阳陵泉、悬钟、关元、手三里、外关、阳溪、梁丘、上巨虚、内庭、滑肉门、大肠俞。2组均每日治疗2次

以临床症状体征、血糖、血脂、尿白蛋白排泄率、尿单核细胞趋化蛋白-1(MCP-1)、肾小球滤过率、肾血流等进行临床疗效评定。结果:调理脾胃针法不仅能改善患者的症状体征

而且对患者的糖、脂代谢和肾小球滤过率、肾血流、尿白蛋白水平都有良性的调节作用

与对照组比较差异有显著性或非常显著性意义(P<0.05

P<0.01)。结论:调理脾胃针法是治疗糖尿病肾病的有效方法

可改善糖、脂代谢异常造成的进行性肾损害

改善肾血流和肾小球的滤过功能

降低尿蛋白排泄率

抑制MCP-1过度表达

保护肾小球、肾小管

从而改善肾功能

延缓肾损害。
