关系极为重要

故历代医家都非常重视它。如淳于意的《诊籍》、韩天爵的《医案一宗》、吴昆的《脉案格式》、喻嘉言的《议病式》等

就是对病历书写进行研究的典范。遗憾的是这些都是用理、法、方药书写病历的模式
