摘要

目的:观察"镇静安神"针法与八脉交会配穴对心脾两虚型失眠患者疗效和脑血流的影响。方法:将60例患者随机分为镇静安神法组(观察组)和八脉交会取穴组(对照组)

各30例。观察组采用"镇静安神法"

针刺四神聪、神门、三阴交;对照组针刺申脉、照海。观察两组患者疗效、临床症状积分及针刺前后TCD检测各项脑血流参数的变化。结果:观察组总有效率为93.3%(28/30)

优于对照组的83.3%(25/30)(P<0.05);两组症状积分治疗后均较治疗前明显改善(均P<0.01)

且观察组治疗后优于对照组(P<0.05);经TCD检测

两组患者治疗后大脑中动脉(MCD)、基底动脉(BA)、椎动脉(VA)的收缩期血流速度峰值(Vp)、舒张期血流速度(Vd)均有所增加(P<0.01

P<0.05);两组治疗后比较

除对左侧椎动脉(LVA)舒张期血流速度改善方面两组差异无统计学意义之外

其余各指标的改善

观察组均优于对照组(P<0.01

P<0.05)。结论:"镇静安神法"、八脉交会取穴法均能有效改善失眠患者的临床症状和体征

并且针刺对失眠的治疗作用与改善失眠患者脑血流状况有关

但"镇静安神法"的治疗效果较八脉交会取穴更为理想。
