摘要

目的:观察针灸并用治疗椎动脉型颈椎病的临床疗效

探讨其作用机制。方法:将60例患者随机分为针刺温灸组和针刺组

每组30例。针刺温灸组针刺百会、风池、颈夹脊穴等

温和灸百会、大椎、命门、神阙、关元穴;针刺组采用单纯针刺治疗

取穴、操作同针刺温灸组中的针刺治疗方法。观察两组患者的疗效、临床症状体征评分及治疗前后行经颅多普勒(TCD)检测椎动脉(VA)、基底动脉(BA)的平均血流速度(Vm)、血流阻力指数(RI)。结果:针刺温灸组总有效率为93.3%(28/30)

优于针刺组的73.3%(22/30)(P<0.05)。两组患者临床症状体征评分治疗后均较治疗前明显改善

且针刺温灸组优于针刺组(P<0.05)。针刺温灸组治疗后VA、BA的Vm、RI均较治疗前明显改善

针刺组VA、BA的Vm亦较治疗前明显改善

且针刺温灸组VA、BA的Vm、RI较针刺组改善更加明显(均P<0.05)。结论:针刺配合温和灸治疗椎动脉型颈椎病疗效较好

能提高VA、BA的平均血流速度

降低血流阻力指数。
