摘要

目的:优选偏头痛针刺即刻镇痛的最佳方案。方法:以36例发作期偏头痛患者为研究对象

采用正交试验设计的方法

应用L9(34)正交表对体穴取穴组合、毫针及电针疗法、耳穴疗法、放血疗法4个因素及其各自不同的3个水平

对发作期偏头痛针刺即刻镇痛效应进行了分析。试验过程中同时应用了随机(分层随机、中心随机)和盲法(评价者盲)的方法。观察时点为治疗前、治疗后10min、20min。疗效评价采用视觉模拟评分法(VAS)。结果:与治疗前相比

在治疗后10min、20min这两个时点

缓解头痛效果最好的针刺方案组合均为:毫针刺法(局部+远端+辨证取穴)+耳穴电针+放血(太阳紫脉或太阳+阿是穴)。结论:在偏头痛发作期

采用局部和远端取穴

再结合辨证取穴进行毫针刺

同时配合耳穴电针以及在太阳紫脉或太阳和阿是穴进行放血治疗

可获得较为理想的即刻镇痛效果。
