摘要

目的:探讨针刺治疗脑卒中后抑郁的疗效和安全性。方法:采用随机双盲对照设计

将目前重性抑郁发作的脑卒中患者43例随机分为观察组(23例)和对照组(20例)。两组患者均给予常规神经内科对症、支持和抗感染治疗

观察组行针刺治疗

穴取百会、印堂、四神聪、太冲等

并口服安慰剂;对照组行非穴点(取位于观察组穴位旁5mm处)浅刺

并口服氟西汀

均连续治疗6周。比较两组Hamilton抑郁量表(HAMD)评分、Asberg抗抑郁药副作用量表(ASES)评分、针刺不良反应情况和疗效。结果:观察组和对照组有效率分别为73.9%(17/23)和80.0%(16/20)

两组疗效相当;两组治疗后HAMD评分较治疗前明显下降(均P<0.05);观察组治疗4周、治疗后ASES评分

对照组治疗后ASES评分均较治疗前明显降低(均P<0.05)

其中治疗4周、治疗后观察组ASES评分较对照组下降更明显(均P<0.05);观察组和对照组针刺不良反应发生率分别为13.0%(3/23)和15.0%(3/20)

均为一过性

对症处理后未再出现。结论:针刺治疗脑卒中后抑郁的疗效与氟西汀相当

没有明显药物治疗引起的不良反应。
