摘要

目的:评定不同频率电针治疗脑卒中下肢偏瘫患者的肌电图效应和对步行能力的影响

为针灸处方提供依据。方法:50例脑卒中下肢偏瘫Brunnstrom分期为Ⅱ-Ⅳ期的患者随机分成3组

即100 Hz组5、0 Hz组2、Hz组。3组分别采用100 Hz5、0 Hz和2 Hz的电针配合常规康复训练进行治疗。治疗前后利用表面肌电图记录患侧膝屈曲和踝背屈时最大等长收缩时主动肌和拮抗肌的肌电活动

并采用Fugl-Meyer评分、临床痉挛指数和Holden步行功能分级对患者的运动功能进行评定。结果:治疗后

100 Hz与50 Hz组股直肌积分肌电值(IEMG)降低(P<0.01

P<0.05)

且比2 Hz组显著(均P<0.05);腓肠肌的IEMG 100 Hz与50 Hz组治疗后也降低(P<0.05

P<0.01)

胫前肌的IEMG仅50 Hz组治疗后增高(P<0.05)。膝屈曲的协同收缩率100 Hz与50 Hz组治疗后均降低(P<0.05

P<0.01)

健患侧的协同收缩比值3组治疗后均显著增高(P<0.01

P<0.05)。踝背屈的协同收缩率3组治疗后均较治疗前显著降低(P<0.05

P<0.01)

健患侧的协同收缩比值仅100 Hz组治疗后差异显著(P<0.01)。3组治疗后Fugl-Meyer功能评分、痉挛指数和步行功能均显著改善(均P<0.01)

但组间差异仅表现为100 Hz组步行能力优于50 Hz与2 Hz组(均P<0.05)。结论:电针配合康复训练有助于降低脑卒中患者下肢的痉挛程度和提高步行功能

50 Hz和100 Hz较2 Hz疗效好

100 Hz的刺激参数可能更佳。
