摘要

目的:验证电针加穴位注射为主治疗术后早期炎性肠梗阻(EPISBO)的疗效

并探讨其作用机制。方法:将116例患者随机分为两组

对照组(56例)予以西医常规治疗包括胃肠减压、肠外营养、抗感染及支持等疗法;观察组(60例)在对照组治疗基础上用电针加穴位注射新斯的明方案治疗

穴取足三里、上巨虚(下巨虚)、大肠俞为主。于治疗前及治疗3天后测定血浆白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNFα-)及血浆胃动素(MOT)含量。结果:观察组平均肠鸣音恢复、肛门排气时间均较对照组时间短(均P<0.01)。两组治疗后IL-6、TNFα-较治疗前下降(均P<0.01)

治疗后观察组IL-6、TNF-α分别为(15.2±4.1)pg/mL、(18.3±6.5)pg/mL

对照组分别为(35.5±7.2)pg/mL、(52.7±12.3)pg/mL

差异有统计学意义(均P<0.01);两组治疗后MOT较治疗前上升(均P<0.01)

治疗后观察组MOT上升程度优于对照组[(190.7±48.5)pg/mL vs(114.6±36.9)pg/mL

P<0.01]。结论:电针加穴位注射为主治疗EPISBO疗效显著

可抑制炎性因子的释放以减轻肠壁水肿

促进MOT的分泌以增强肠蠕动

从而促进胃肠功能的恢复。
