摘要

目的:评价五脏俞加膈俞为主辨证治疗原发性抑郁症的疗效。方法:将60例原发性抑郁症患者随机分为针刺组(31例)和西药组(29例)

针刺组以五脏俞加膈俞辨证加减

肝郁脾虚者配太冲、三阴交

心脾两虚者配神门、三阴交;西药组口服盐酸氟西汀胶囊20mg

每日1次

均治疗6周。治疗2、4、6周后分别对汉密尔顿抑郁(HAMD)量表、HAMD因子分析、Asberg副作用量表进行评分

对比两组临床疗效。结果:治疗6周后

针刺组总有效率为67.7%(21/31)

西药组为65.5%(19/29)

两组疗效相近(P>0.05)。两组治疗2、4、6周后HAMD评分、焦虑躯体化因子、睡眠因子、迟缓因子评分均较治疗前明显下降(均P<0.01);两组组间治疗后各时间点比较

针刺组焦虑躯体化因子、睡眠因子较西药组改善明显(均P<0.05)

西药组迟缓因子较针刺组改善明显(P<0.05)。西药组Asberg量表评分在2周时达到高峰

之后呈缓慢下降趋势

针刺组副作用趋势低平

治疗6周后比较总体低于西药组(P<0.05)。结论:辨证治疗抑郁症的针灸经验方与口服盐酸氟西汀胶囊近期疗效相当

副作用较盐酸氟西汀胶囊少。
