摘要

目的:观察针刺与喷吸沙丁胺醇对支气管哮喘急性发作患者的即刻平喘效果差异

并比较留针过程中不同时间点的疗效差异。方法:将577例患者随机分为两组

针刺组(289例)和喷吸组(288例)。所有患者均行吸氧、阿奇霉素抗感染治疗。患者哮喘急性发作时

针刺组立即取鱼际

针尖向掌心方向斜刺20～35mm

得气后留针60min

每5分钟捻转行针1次;喷吸组喷吸沙丁胺醇气雾剂200μg

24h少于8喷。观察急性发作时及针刺组针刺后即刻、留针5min、30min、60min

喷吸组喷吸后5min肺功能指标、中医症状评分变化

并比较组间差异。结果:针刺组针刺后即刻与喷吸组喷吸后5min肺功能指标、中医症状评分均较治疗前明显改善(均P<0.05)

喷吸组优于针刺组(均P<0.05);针刺组留针5min与针刺后即刻无明显差异;针刺组留针30min以上指标较针刺即刻明显改善(均P<0.05)

与喷吸组疗效相当(均P>0.05);针刺组留针60min与留针30min以上指标差异无统计学意义(均P>0.05)。结论:针刺鱼际对支气管哮喘急性发作具有平喘功效

即刻起效快

留针30min达最佳疗效

与喷吸沙丁胺醇疗效相当。
