摘要

目的:探讨针刺麻醉对全麻下老年患者肠癌切除术后认知功能障碍(POCD)的影响及作用机制。方法:将择期全麻下行肠癌切除的老年患者75例随机分为针刺加全麻组(A组

38例)、单纯全麻组(B组

37例)。A组患者先行针刺麻醉

电针百会、内关、足三里、三阴交

诱导20min后行气管内全身麻醉

电针刺激至手术结束。B组只行单纯气管内全身麻醉。运用简易智能量表(MMSE)评定并记录两组患者术前1天、术后第3天的认知功能变化;监测患者术前、术毕S-100β蛋白水平变化。结果:A组患者术后麻醉苏醒时间较B组缩短[(20.35±6.05)min vs(28.24±7.68)min

P<0.05];第3天POCD的发生率A组低于B组[23.7%(9/38)vs 35.1%(13/37)

P<0.05];两组患者术毕血清S-100β蛋白水平较术前升高(均P<0.05)

术毕A组血清S-100β蛋白水平低于B组(P<0.05)。结论:针刺麻醉可以减少老年患者肠癌切除术后认知功能障碍的发生并降低S-100β蛋白水平。
