摘要

目的:对比电针断续波、疏密波、连续波联合康复训练治疗脑卒中后肩关节半脱位的疗效差异

为电针波型的选择提供临床依据。方法:将75例肩关节半脱位患者随机分为断续波组、疏密波组、连续波组

每组25例。3组均在控制血压、血糖等常规内科治疗基础上

予以电针疗法和康复训练。电针穴取肩前、抬肩、臂臑、肩中、肩贞、臑俞、肩外俞、秉风

断续波组采用断续波

频率15Hz;疏密波组采用疏密波

频率2Hz/15Hz;连续波组采用连续波

频率15Hz

均电针刺激20min

每天1次

每周6次

治疗4周。分别于治疗前后采用Fugl-Meyer评分和Barthel指数评分评定患侧肩关节功能恢复情况

并比较3组疗效。结果:断续波组总有效率为96.0%(24/25)

优于疏密波组的68.0%(17/25)及连续波组的64.0%(16/25)(均P<0.05);治疗后3组Fuglmeyer评分和Barthel指数评分均显著改善(均P<0.05)

且断续波组优于其他两组(均P<0.01)。结论:电针断续波联合康复训练治疗脑卒中后肩关节半脱位效果优于电针疏密波和连续波

能够有效改善肩关节半脱位

促进肩关节功能恢复。
