摘要

目的:评价针刺治疗膝关节骨性关节炎临床疗效及疗效持续时间。方法:采用同期非随机对照试验

193例膝关节骨性关节炎患者分别进入即刻针刺组(A组

97例)和随后针刺组(B组

96例)。A组给予4周的针刺治疗

治疗结束后随访4周。B组等待4周后

再给予4周同样的针刺治疗。两组患者取穴相同

均为梁丘、犊鼻、足三里、阳陵泉、阴陵泉、血海、膝眼、膝阳关。主要评价指标选用西安大略麦马斯特大学骨性关节炎指数(WOMAC)

次要评价指标选用疼痛视觉评分表(VAS)

在试验开始、第4周和第8周由患者完成。两组各有72例患者完成试验

脱失患者资料进入最终数据分析。结果:在试验4周时

A组WOMAC总分为25.8±22.0

B组为43.8±22.2

两组间的差异有统计学意义(P<0.001);A组VAS得分为31.8±24.6

B组为56.6±25.8

差异亦有统计学意义(P<0.001)。8周后

A组随访发现疗效略有下降

但与治疗前比较仍有明显的改善。结论:针刺治疗对关节疼痛程度和关节功能有明显改善

停止治疗4周后针灸疗效仍持续存在。
