摘要

目的:采用正交设计方法

优化针灸与运动疗法对肩周炎的治疗方案。方法:将192例肩周炎患者按分层的原则采用L8(27)的正交设计表随机分为8组

每组24例

分别采用针刺时机(A因素:A1急性期

A2粘连期)、选穴配伍(B因素:B1局部取穴

B2局部取穴配合循经远道取穴)、毫针及温针灸疗法(C因素:C1毫针针刺

C2毫针针刺结合温针灸)、主动功能锻炼(D因素:D1不进行主动功能锻炼

D2进行主动功能锻炼)4个因素及其各自不同的2水平进行治疗

每天治疗1次

10次为一疗程

共治疗2个疗程。观察时点为治疗1个疗程后、2个疗程后。评价采用简式McGill疼痛量表(MPQ)及肩关节活动障碍评分。结果:正交设计分析表明

考虑分层因素

年龄是影响患者肩部疼痛评定、肩功能活动障碍评定的主要因素(P<0.01)

并且患者肩功能等级对疼痛评定、肩功能活动障碍疗效有明显的影响(P<0.01);4因素2水平的最佳搭配方案为A1B1C2D2、A2B2C2D2

通过SAS统计分析表明

在急性期和粘连期C2D2即毫针针刺结合温针灸和主动功能锻炼是影响患者肩功能活动改善的主要因素(P<0.05)。结论:肩周炎急性期

采用局部取穴、毫针针刺结合温针灸与主动功能锻炼

慢性期局部取穴联合远端取穴、毫针针刺结合温针灸与主动功能锻炼为最佳治疗方案

并且在临床治疗中

需综合考虑患者年龄、性别、肩关节功能状态与疗程对疗效的影响。
