摘要

目的:探讨针刺加叩刺治疗肥胖并发高脂血症肝郁脾虚型患者的疗效及其作用机制。方法:将104例女性患者随机分为针叩组和针刺组

每组52例。针刺组采用针刺期门、太冲、章门、太白、足三里、膈俞、肝俞、脾俞等穴;针叩组在针刺组治疗基础上

采用梅花针叩刺各穴。两组均隔日治疗1次

持续治疗3个月。观察两组患者治疗前后肥胖指标:体质量、体质指数(BMI)、体脂百分率(F%)、肥胖度(A)

血脂水平:总胆固醇(TC)、三酰甘油(TG)、低密度脂蛋白(LDL)、高密度脂蛋白(HDL)

脂肪-胰岛轴相关指标:空腹血糖(FBS)、空腹瘦素(FLP)、空腹胰岛素(FINS)、胰岛素敏感指数(ISI)、胰岛素抵抗指数(Homa-IR)、胰岛素分泌指数(Homa-β)以及自主神经功能指数(Y值)的变化

并比较两组疗效。结果:针叩组、针刺组总有效率分别为96.2%(50/52)、84.6%(44/52)

组间差异无统计学意义(P>0.05);两组治疗后各项肥胖指标、血脂指标、脂肪-胰岛轴相关指标、自主神经功能指数较治疗前均改善(P<0.01

P<0.05)

且针叩组改善更显著(P<0.01

P<0.05)。结论:针刺加叩刺治疗肥胖并发高脂血症具有减肥降脂的双重作用

其作用机制可能与良性调整患者血糖、脂质代谢及脂肪-胰岛轴有关。
