摘要

目的:观察针刺与艾灸对体外受精-胚胎移植(IVF-ET)者子宫内膜及妊娠结局的影响

探讨针灸在IVF-ET过程中的应用价值。方法:选择经黄体期标准长方案治疗的IVF-ET者114例

随机分为观察组和对照组

各57例。观察组在促排卵时开始灸神阙穴

针刺中极、关元、气海、子宫、血海等穴位至移植日止

此为一疗程

共治疗3个疗程;对照组不加用针灸干预。观察两组受试者的子宫内膜形态特征、内膜下血流指数、注射人绒毛膜促性腺激素(hCG)日血清雌二醇(E2)、孕激素(P)、黄体生成素(LH)水平及促性腺激素(Gn)用量、时间、获卵数、优质胚胎数、胚胎种植率、临床妊娠率。结果:观察组hCG日A型内膜比例、优质胚胎率高于对照组

差异有统计学意义(均P<0.05);两组hCG日子宫内膜的厚度差异无统计学意义(P>0.05);观察组子宫内膜血流动力学指数(收缩期最高血流速度/舒张期末血流速度

peak systolic blood velocity/end-diastolic blood velocity

S/D)、阻力指数(resistive index

RI)、搏动指数(pulse index

PI)均低于对照组(P<0.01

P<0.05);观察组hCG日血清E2、P水平高于对照组(均P<0.05);两组Gn用量、Gn用药时间、hCG日≥1.6cm卵泡数、获卵数、胚胎种植率和临床妊娠率、hCG日血清LH水平

差异无统计学意义(均P>0.05)。结论:IVF-ET治疗中针灸可以影响hCG日雌孕激素水平

提高优胚率

改善子宫内膜的血流状态和内膜形态

从而改善子宫内膜容受性

有望成为改善IVF-ET治疗结局的辅助治疗手段。
