摘要

患者

男

54岁

因"右侧肢体无力伴言语不清1h"于2012年2月15日住院。查体:BP:180/100mmHg(1mmHg≈0.133kPa)。神志清

部分混合性失语

右肢肌力Ⅲ级

右侧巴氏征阳性。既往高血压病史10年。头CT示:左侧外囊区出血约20mL(多田氏算法)

中线结构居中;右侧基底节区腔隙性脑梗死。诊断左侧外囊区出血

予内科治疗。入院12h复查头CT示出血量达35mL

中线向右移位约3mm。急诊全麻下行脑内血肿清除、去骨瓣减压术。术后患者神清

不完全性混合性失语

右上肢肌力Ⅲ级

右下肢肌力Ⅳ-级。术后第1、7、11天复查CT

术后第14天查MRI均无再出血
