摘要

目的:比较低频电穴位刺激与常规西药治疗脑卒中后失眠患者的临床疗效

并探讨其作用机制。方法:将120例脑卒中后失眠患者随机分为低频电刺激组、药物组及安慰剂组

每组40例。低频电刺激组采用低频脉冲电治疗仪对大椎穴、肾俞穴进行刺激

每天1次

15天为一疗程

治疗2个疗程。药物组口服艾司唑仑

每次1mg;安慰剂组口服淀粉胶囊

每次1粒;两组均每晚睡前服用

连服15天为一疗程

均治疗2个疗程。观察3组患者治疗前后血浆5-羟色胺(5-HT)和去甲肾上腺素(NE)的含量

并比较各组疗效。结果:安慰剂组剔除1例

低频电刺激组、药物组和安慰剂组总有效率分别为95.0%(38/40)、92.5%(37/40)和17.9%(7/39)

低频电刺激组、药物组疗效均明显优于安慰剂组(均P<0.01)

低频电刺激组和药物组疗效比较差异无统计学意义(P>0.05)。安慰剂组治疗前后血浆5-HT、NE的含量变化差异无统计学意义(均P>0.05);低频电刺激组和药物组患者血浆5-HT含量较治疗前均增高(均P<0.05)

NE含量较治疗前均降低(均P<0.05)

两组间比较差异无统计学意义(均P>0.05)。结论:低频电穴位刺激治疗脑卒中后失眠安全有效

与口服艾司唑仑疗效相当

升高血浆5-HT含量、降低血浆NE含量可能是其作用机制之一。
