摘要

目的:评价梅花针关键点叩刺结合Bobath疗法治疗脑梗死后偏瘫痉挛状态的疗效。方法:收集符合纳入标准的脑梗死后偏瘫痉挛状态患者80例

随机分为观察组和对照组

每组40例。对照组运用Bobath缓解痉挛手法治疗

治疗8周;观察组在对照组治疗的基础上加梅花针关键点叩刺

穴取肩髃、肩髎、肩贞、合谷、承扶、足三里、膝阳关等穴

每次15min

每日1次。分别于治疗前、治疗4周后、治疗8周后采用Fugl-Meyer评分法(FMA)、Barthel Index(BI)评定两组患者肢体运动功能及日常生活活动能力

采用改良Ashworth量表评定抗痉挛疗效。结果:治疗4周、8周后

两组FMA评分、BI评分均显著高于治疗前(均P<0.05)

且于治疗8周后观察组显著优于对照组(均P<0.05)。治疗4周、8周后两组患者痉挛状态分级均较治疗前改善(均P<0.05)

且于8周后观察组优于对照组(P<0.05)。观察组治疗8周后抗痉挛总有效率为90.0%(36/40)

优于对照组的75.0%(30/40

P<0.05)。结论:梅花针关键点叩刺结合Bobath疗法可有效缓解脑梗死偏瘫患者的痉挛状态

改善肢体功能

提高日常生活活动能力。
