摘要

目的:比较颈部穴位结合腰部穴位温针灸与常规颈部穴位温针灸治疗颈型颈椎病的近远期疗效差异。方法:将81例颈型颈椎病患者随机分为A组(41例)和B组(40例)

其中脱失失访病例2例

最后A组40例、B组39例完成试验。A组采用常规颈部穴位结合腰部穴温针灸治疗

颈部穴取风池、天柱、颈百劳、完骨、天牖、阿是穴(包括压痛点或触诊有硬结或条索状物处)

腰部取大肠俞、气海俞、腰5夹脊穴

于风池、天柱、大肠俞行温针灸;B组采用常规颈部穴位温针灸治疗

取穴同A组中的颈部取穴。两组均隔日治疗1次

每周治疗3次

于治疗前、治疗1周时、治疗2周结束时、治疗结束1个月随访时记录各个时间点的颈痛量表(NPQ)评分、颈椎活动度(ROM)评分、颈部症状积分

并评定治疗结束时和随访时的疗效。结果:与治疗前比较

两组治疗后各个观察时点各项评分均显著改善(均P<0.05)

随访时颈痛NPQ评分、颈椎ROM评分、颈部症状积分组间比较差异有统计学意义(均P<0.05)

A组优于B组。A组治疗2周结束时总有效率为92.5%(37/40)

B组为87.2%(34/39

P>0.05);随访时A组总有效率为87.5%(35/40)

优于B组的64.1%(25/39

P<0.05)。结论:"颈腰同治"温针灸及常规穴位温针灸治疗颈型颈椎病都有一定的近期疗效;"颈腰同治"温针灸治疗颈型颈椎病在远期疗效上优于常规颈部穴位温针灸者。
