摘要

目的:观察电针合雷火灸疗法与单纯电针治疗寒湿痹阻型退行性膝关节炎的临床疗效差异。方法:将80例寒湿痹阻型退行性膝关节炎患者随机分为电针加雷火灸组(针灸组

40例)和电针组(40例)。电针组针刺取穴以阿是穴、内膝眼、足三里、犊鼻、阳陵泉、阴陵泉、梁丘为主穴

得气后连接电针治疗仪

留针25min

每日1次。针灸组针刺取穴及电针方法与电针组相同

并在电针后采用雷火灸以悬灸方式灸阿是穴、犊鼻、梁丘

每日针灸1次

每次20min。两组均10d为一疗程

连续治疗2个疗程。观察两组治疗前后疼痛视觉模拟量表(VAS)评分、Lequesne指数评分、Lysholm膝关节评分、血清C-反应蛋白(CRP)及血沉(ESR)指标变化情况

评定临床疗效并随访6个月统计复发率。结果:治疗后针灸组总有效率为95.0%(38/40)

高于电针组的82.5%(33/40

P<0.05);两组治疗后Lequesne指数评分、VAS评分、血清CRP和ESR水平均较治疗前明显降低(均P<0.05)

针灸组治疗后两项评分及血清CRP及ESR水平均明显低于电针组(均P<0.05);两组治疗后Lysholm膝关节评分评级及优良率均较治疗前提高(均P<0.05)

针灸组治疗后优良率明显高于电针组[87.5%(35/40)vs 72.5%(29/40)

P<0.05];随访6个月

针灸组复发率低于电针组[26.7%(4/15)vs 100.0%(9/9)

P<0.05]。结论:电针联合雷火灸治疗寒湿痹阻型退行性膝关节炎临床疗效显著

与单纯电针相比

电针联合雷火灸在疗效持久性方面更具有优势。
