摘要

目的:比较"督穴导气针法"联合电针与常规电针治疗血瘀型轻中度腰椎间盘突出症的临床疗效差异。方法:将201例血瘀型轻中度腰椎间盘突出症患者随机分为观察组(101例)与对照组(100例)。观察组采用"督穴导气针法"联合电针治疗

穴取腰阳关、十七椎、环跳和阳陵泉

腰阳关、十七椎在得气基础上行"导气针法";环跳、阳陵泉得气后实施电针疗法。隔天治疗1次

每周3次

12次为一疗程

共观察治疗1个疗程;对照组取穴同观察组

4穴分别行电针治疗

电针参数、治疗时间与周期和观察组相同。比较两组患者治疗前、治疗后及治疗后3个月随访时日本骨科学会下腰痛评分(JOA评分)

治疗前后腰部红外热像图温度变化及静息状态下肌肉压力-位移曲线下面积(RAUC)

并比较两组患者近、远期疗效。结果:观察组近、远期总有效率分别为89.11%(90/101)、91.11%(82/90)

对照组分别为86.00%(86/100)、89.53%(77/86)

组间比较差异均无统计学意义(均P>0.05);观察组近、远期临床治愈率分别为52.48%(53/101)、55.56%(50/90)

均优于对照组的36.00%(36/100)、34.88%(30/86

均P<0.05)。与治疗前比较

两组治疗后及随访时JOA评分均提高(均P<0.01)

各时点组间差异均无统计学意义(均P>0.05);两组治疗后腰部红外热像图温度均升高(均P<0.01)

且升高幅度观察组大于对照组(P<0.01);观察组治疗后腰部RAUC较治疗前明显增大(P<0.01)

且大于对照组(P<0.01)。结论:"督穴导气针法"联合电针治疗血瘀型轻中度腰椎间盘突出症临床疗效优于常规电针治疗。
