摘要

目的:比较远道巨刺结合局部针刺与单纯局部针刺治疗轻、中度腕管综合征的临床疗效差异。方法:将60例轻、中度腕管综合征患者按随机数字表分为观察组与对照组

每组30例。两组针刺同取患侧劳宫、大陵、内关、鱼际、合谷;观察组先刺对侧太冲、中封

行提插泻法

再取患侧所选穴位;对照组仅针刺患侧所选穴位

两组患侧穴位均施平补平泻手法

每日治疗1次

每次30min

10d为一疗程

3个疗程后行疗效评价。观察两组治疗前后正中神经的神经电生理改变、Levine腕管综合征问卷调查表及腕关节患者评估量表(patient-rated wrist evaluation

PRWE)评分变化并评价疗效。结果:观察组总有效率为90.0%(27/30)

优于对照组的70.0%(21/30

P<0.05);两组治疗后正中神经感觉传导速度和波幅均增高

差异有统计学意义(均P<0.05)

且观察组较对照组增高明显(均P<0.05);两组治疗后症状严重程度(SSS)评分和功能状态(FSS)评分均明显下降(均P<0.05)

且观察组治疗后SSS、FSS评分明显低于对照组(均P<0.05);两组治疗后PRWE评分较治疗前明显降低(均P<0.05)

且观察组治疗后低于对照组(P<0.05)。结论:远道巨刺结合局部针刺治疗轻、中度腕管综合征疗效确切

明显优于常规局部针刺。
