摘要

目的:比较"截瘫三联针法"与常规针刺法治疗脊髓损伤患者心理与日常生活活动能力的临床疗效差异。方法:将50例脊髓损伤患者随机分为观察组与对照组

每组25例。两组均采用康复训练

观察组在此基础上加用"截瘫三联针"

即分别在损伤平面的上下两节段针刺督脉与背俞穴

并通过外周神经电刺激仪定位双下肢关键肌运动点

定位完成后损伤部位上下两节段督脉穴、背俞穴、关键肌运动点均给予电针治疗;对照组采用常规电针

穴取损伤部位上下两节段督脉穴、背俞穴

下肢取环跳、足三里、悬钟、阳陵泉。两组均每日治疗1次

1个月为一疗程

共2个疗程。另选取就诊于成都军区总医院体检中心的健康体检者25例

作为正常对照组。治疗前、治疗1、2个疗程后清晨分别检测观察组与对照组患者外周血清5-羟色胺(5-HT)含量

用改良Barthel指数(MBI)观察患者日常生活活动能力

汉密尔顿抑郁量表(HAMD)、汉密尔顿焦虑量表(HAMA)进行心理状态评定。结果:(1)治疗前观察组、对照组血清5-HT含量均低于正常对照组(均P<0.05)。(2)与治疗前相比

治疗1个疗程后两组患者MBI评分、血清5-HT含量均上升

HAMA、HAMD评分均下降(均P<0.05)

且观察组改善优于对照组(P<0.05);治疗2个疗程后两组患者MBI评分、血清5-HT含量均较治疗前上升

HAMA、HAMD评分均下降(均P<0.05)

但两组间比较差异无统计学意义(均P>0.05)。(3)经Pearson相关性分析

脊髓损伤患者血清5-HT含量与病程、HAMD与HAMA评分呈负相关(均P<0.05)

与MBI评分呈正相关(P<0.05)。结论:脊髓损伤患者外周血清5-HT含量较正常人降低

外周血检测5-HT可早期提示脊髓损伤患者抑郁与焦虑状态。"截瘫三联针法"较传统电针治疗可在早期改善抑郁、焦虑状态

综合提高脊髓损伤患者日常生活能力。
