摘要

目的:观察针刺中髎、下髎对出口梗阻型便秘两类亚型的疗效差异。方法:回顾性分析87例出口梗阻型便秘患者

其中盆底失弛缓型61例

盆底松弛型26例。采用0.30 mm×75 mm针灸针针刺中髎、下髎

垂直于骶后孔进针60～75 mm

并接电针(疏密波

频率为2 Hz/15 Hz)。每日1次

每次30 min

每周治疗5 d

连续治疗2周。观察两类亚型出口梗阻型便秘患者治疗前后便秘临床症状评分、生存质量自评量表(PAC-QOL)评分情况。结果:两类亚型治疗后便秘临床症状总评分

便质、排便频率、排便花费时间、排便费力程度、排便不尽感、肛门坠胀感、腹胀评分均较治疗前降低(均P<0.01)

松弛型腹痛评分治疗后较治疗前降低(P<0.01)

失弛缓型腹痛评分治疗前后比较差异无统计学意义(P>0.05);两类亚型排便时间评分治疗前后差值比较

失弛缓型大于松弛型(P<0.05)

肛门坠胀感评分治疗前后差值比较

松弛型大于失弛缓型(P<0.05)。两类亚型治疗后PAC-QOL评分均较治疗前降低(均P<0.01)

两类亚型治疗前后差值比较差异无统计学意义(P>0.05)。结论:同一电针方案对两类亚型出口梗阻型便秘均有效

在改善排便时间方面

失弛缓型优于松弛型

在改善肛门坠胀感方面

松弛型优于失弛缓型。
