摘要

目的:观察快捻久留针刺法对后循环缺血眩晕大鼠前庭神经核细胞凋亡及半胱氨酸天冬氨酸蛋白酶-3(Caspase-3)、B淋巴细胞瘤-2(Bcl-2)、B淋巴细胞因子相关X蛋白(Bax)表达的影响。方法:将70只健康SD大鼠随机分为假手术组、模型组、药物组、普通针刺组、快捻久留组

每组14只。除假手术组外各组大鼠给予手术结扎右侧颈总动脉(CCA)和右侧锁骨下动脉(SCA)建立后循环缺血眩晕模型

假手术组剥离右侧CCA和右侧SCA

不进行结扎

直接缝合。药物组给予盐酸氟桂利嗪混悬液(10 mL/kg)灌胃;两针刺组均选取"百会""率谷""风池"穴进行针刺

普通针刺组给予常规针刺并留针30min;快捻久留组常规针刺后行频率200～300次/min的快速捻转1 min

并留针60 min;假手术组和模型组不予干预。干预均为每日1次

共干预10 d。观察各组大鼠前庭神经核局部血流下降率;采用TUNEL法观察大鼠前庭神经核细胞凋亡情况;免疫组化法检测凋亡相关基因Caspase-3、Bcl-2、Bax蛋白的表达。结果:与假手术组比较

模型组大鼠右侧前庭神经核局部血流下降率明显升高(P<0.01)

前庭神经核细胞凋亡指数(AI)明显升高(P<0.01);与模型组比较

两针刺组及药物组大鼠右侧前庭神经核局部血流下降率显著降低(P<0.01)

前庭神经核细胞AI明显降低(P<0.01);快捻久留组大鼠右侧前庭神经核局部血流下降率低于药物组和普通针刺组(P<0.01

P<0.05)

前庭神经核细胞AI低于普通针刺组和药物组(P<0.05)。与假手术组比较

模型组大鼠前庭神经核Bcl-2蛋白表达明显降低(P<0.01)

Bax、Caspase-3蛋白表达明显升高(P<0.01);与模型组比较

两针刺组及药物组大鼠前庭神经核Bcl-2蛋白表达明显升高(P<0.01)

Bax、Caspase-3蛋白表达明显降低(P<0.01);快捻久留组Bcl-2蛋白表达高于普通针刺组和药物组(P<0.05)

Bax、Caspase-3蛋白表达低于普通针刺组和药物组(P<0.05)。结论:快捻久留针刺法能有效抑制后循环缺血眩晕大鼠前庭神经核的细胞凋亡

其机制可能与改善前庭神经核供血

调节Caspase-3、Bcl-2、Bax蛋白表达有关。
