摘要

目的:探讨针刺次髎穴对经阴道分娩高龄产妇产后尿潴留的预防作用

及对产后首次排尿时间及尿量的影响。方法:将180例经阴道分娩高龄产妇(≥35岁)随机分为空白对照组、条件对照组和观察组

每组60例。空白对照组予以常规护理方案;条件对照组在空白对照组基础上采取热毛巾外敷膀胱处、手指按压刺激排尿等干预措施;观察组在空白对照组基础上

于产后1 h针刺双侧次髎穴

留针20 min

仅治疗1次。观察3组高龄产妇产后尿潴留发生率

未发生尿潴留产妇首次排尿时间及尿量

并统计3组出院满意率。结果:与空白及条件两对照组比较

观察组高龄产妇的尿潴留发生率降低[5.0%(3/60)vs26.7%(16/60)

P<0.01;5.0%(3/60)vs16.7%(10/60)

P<0.05];条件对照组尿潴留发生率低于空白对照组[16.7%(10/60)vs 26.7%(16/60)

P<0.05]。未发生尿潴留的高龄产妇中

观察组首次排尿时间明显早于空白对照组和条件对照组(P<0.01)

条件对照组早于空白对照组(P<0.01);观察组首次排尿尿量大于空白对照组和条件对照组(P<0.05

P<0.01)。观察组、条件对照组的出院满意率均优于空白对照组(P<0.01)。结论:针刺次髎穴可有效预防经阴道分娩高龄产妇产后尿潴留的发生

提早产后首次排尿时间

增加首次排尿尿量。
