摘要

目的:观察针刺合募配穴联合痛泻要方治疗肝郁脾虚证腹泻型肠易激综合征(IBS-D)的临床疗效

并探讨其可能的作用机制。方法:将123例肝郁脾虚证IBS-D患者随机分为针药组、中药组和西药组

每组41例。中药组口服痛泻要方

每天3次;针药组口服痛泻要方联合针刺治疗

穴取上巨虚、天枢、太冲、三阴交、足三里常规针刺

每天1次;西药组口服匹维溴铵片

每次50mg

每日3次

各组均连续治疗4周。比较各组患者治疗前后中医证候评分、焦虑自评量表(SAS)评分、抑郁自评量表(SDS)评分

血清降钙素基因相关肽(CGRP)、血管活性肽(VIP)及MAPK信号通路指标ERK1 mRNA、ERK2 mRNA表达水平

并比较治疗后临床疗效。结果:治疗后

各组患者中医证候总评分、SAS与SDS评分均显著降低(P<0.05)

针药组低于中药组、西药组(P<0.05);针药组总有效率高于中药组及西药组[87.8%(36/41)vs 78.0%(32/41)

68.3%(28/41)

P<0.05];治疗后

各组患者血清CGRP、VIP水平均下降(P<0.05)

针药组低于中药组、西药组(P<0.05);治疗后

各组患者ERK1mRNA、ERK2mRNA表达水平均降低(P<0.05)

针药组表达水平低于西药组(P<0.05)。结论:针刺合募配穴联合痛泻要方可有效缓解肝郁脾虚证IBS-D患者临床症状

改善焦虑、抑郁状态

其机制可能与调控MAPK信号通路中ERK1 mRNA、ERK2 mRNA表达

降低血清CGRP、VIP水平有关。
