摘要

患者

男

69岁

2019年10月18日初诊。主诉:餐后血压降低1月余。现病史:5年前因出现行走时脚踩棉絮感、运动欠协调、头晕、汗出间作等症状

就诊于某三甲医院

考虑多系统萎缩、小脑共济失调。先后于多家医院进行中西医综合治疗

病情进展缓慢。1年前出现语速减慢、吐字不清

尿频尿急尿少、排尿困难

予留置尿管。1个月半前无明显诱因出现餐后尤其早餐后血压降低

最低至50/40 mmHg(1 mmHg=0.133 kPa)

伴头晕、眼前黑朦后意识丧失

平躺10 min后意识转清、上述症状缓解、血压回升。
