摘要

目的:比较分期针灸法与芬吗通治疗早发性卵巢功能不全(POI)的疗效。方法:将66例POI患者随机分为观察组(33例

脱落3例)和对照组(33例

脱落2例)。观察组基于"阴阳消长转化"理论

于经后期、排卵期、经前期、行经期采用分期针灸法治疗

隔日1次

每周治疗3次。对照组口服芬吗通(雌二醇片/雌二醇地屈孕酮片

每日1片)治疗。两组均连续治疗3个月经周期。观察两组患者治疗前后卵巢功能[血清卵泡刺激素(FSH)、促黄体生成素(LH)、雌二醇(E_2)、抗苗勒管激素(AMH)水平及窦卵泡计数(AFC)]及围绝经期症状[改良Kupperman指数(KI)量表评分]

并比较两组患者治疗后的月经改善情况。结果:与治疗前比较

两组患者治疗后血清FSH、LH水平下降(P<0.01)

E_2水平升高(P<0.01);观察组患者治疗后血清AMH水平及AFC升高(P<0.01)。观察组患者治疗后血清AMH水平及AFC均高于对照组(P<0.05)。治疗后两组闭经患者月经回潮率及患者月经规律率比较差异均无统计学意义(P>0.05)。治疗后两组患者改良KI量表评分均较治疗前降低(P<0.01)

且观察组低于对照组(P<0.05)。结论:基于"阴阳消长转化"理论的分期针灸法在改善POI患者血清性激素水平及月经症状方面与芬吗通疗效相似

在提高血清AMH水平、AFC及改善围绝经期症状方面较芬吗通具有优势。
