摘要

目的:验证运用程氏"通脱法"理论指导浮针治疗椎动脉型颈椎病(CSA)的优效性。方法:将150例CSA患者随机分为通脱法浮针组、通脱法针刺组、常规浮针组

每组50例。通脱法浮针组在"通脱法"理论指导下予浮针治疗

先在前臂背侧远端肌筋膜激发点(MTrP)行远端"通"法

再在颈部肌群MTrP行近端"脱"法;通脱法针刺组先循经远端取穴(内关、曲池、太冲、行间等)针刺

再在近端取风池、百会穴针刺;常规浮针组仅在颈部肌群MTrP行浮针治疗。各组均每日1次

3 d为一疗程

治疗2个疗程。于治疗前后观察各组颈性眩晕症状与功能评估量表评分、椎动脉收缩期血流峰值流速(PSV)和阻力指数(RI)。结果:治疗后

各组患者颈性眩晕症状与功能评估量表评分、PSV均较治疗前升高(P<0.01)

RI较治疗前降低(P<0.01)

且通脱法浮针组颈性眩晕症状与功能评估量表评分、PSV高于其余两组(P<0.01)

RI低于其余两组(P<0.01)。结论:"通脱法"理论指导浮针治疗CSA能改善患者的颈性眩晕症状及椎动脉血流状态

疗效优于"通脱法"针刺和常规浮针治疗。
