摘要

目的:比较王居易经络诊察法联合Bobath康复训练与单纯Bobath康复训练治疗脑卒中后肩手综合征(SHS)Ⅰ期的临床疗效。方法:将106例脑卒中后SHSⅠ期患者随机分为观察组(53例

脱落2例)和对照组(53例

脱落3例)。两组均予药物控制基础病及常规针刺治疗(穴取患侧外关、手三里、肩髃)。对照组予Bobath康复训练治疗

每次20 min;观察组在对照组基础上

采用王居易经络诊察法于患侧手太阴与手阳明经经络异常处行经络调整手法

每次20 min。两组均每天治疗1次

每周5次

共治疗8周。分别于治疗前后及治疗后6周随访时记录两组患者视觉模拟量表(VAS)、上肢Fugl-Meyer运动功能量表(FMA)及Barthel指数(BI)评分

并于治疗后评定两组临床疗效。结果:治疗后及随访时

两组患者VAS评分均较前降低(P<0.05)

FMA、BI评分均较治疗前升高(P<0.05);观察组患者VAS评分低于对照组(P<0.05)

FMA、BI评分高于对照组(P<0.05)。观察组总有效率为82.4%(42/51)

高于对照组的62.0%(31/50

P<0.05)。结论:王居易经络诊察法联合Bobath康复训练可有效治疗脑卒中后SHSⅠ期患者

减轻疼痛症状及关节运动功能障碍

改善生活质量

其疗效优于单纯Bobath康复训练。
