摘要

患者，女，44岁，于2021年8月16日就诊。主诉：双手指甲脱落伴疼痛2个月，加重3 d。现病史：2个月前自觉手指冰凉，渐至双手指端红紫，指甲脱落，疼痛、麻木加重，就诊于当地某医院，诊断为“雷诺综合征”，口服硝苯地平缓释片Ⅰ（每天2次，每次10 mg），治疗无效。刻下症：双手指末端皮肤变硬、颜色红紫，指甲脱落（图1A），患指冰冷，遇冷后疼痛难忍，纳可，二便调，舌淡紫、苔白腻，脉弦细。
