摘要

目的

观察针刺水通、水金穴预防全身麻醉诱导期舒芬太尼诱发呛咳反应的疗效及对血流动力学的影响。

方法

将80例在全身麻醉下行择期手术患者，随机分为观察组（40例）和对照组（40例，剔除1例）。对照组给予常规麻醉诱导，静脉注射1%枸橼酸舒芬太尼注射液0.5 μg/kg、1%丙泊酚注射液（总剂量按照2 mg/kg计算）、苯磺顺阿曲库铵注射液0.2 mg/kg；观察组在常规麻醉诱导前针刺右侧水通、水金穴，留针30 min。观察麻醉诱导期间两组患者呛咳、胸壁僵硬并发症情况，记录患者进入手术室平躺5 min后（T

）、静脉注射枸橼酸舒芬太尼注射液即刻（T

）及注射后2 min（T

）、插管前1 min（T

）、插管1 min后（T

）的收缩压（SBP）、心率（HR）、脉搏血氧饱和度（SpO

）。

结果

麻醉诱导期，观察组患者轻、中、重度呛咳情况优于对照组（

0.05），呛咳总例数及总发生率均低于对照组（

0.05）。两组患者各有2例胸壁僵硬发生，差异无统计学意义（

0.05）。两组患者T

时SBP、HR、SpO

比较，差异无统计学意义（

0.05）。与T

时比较，对照组T

时SBP、HR升高（

结论

针刺水通、水金穴可以有效预防舒芬太尼引起的呛咳反应，降低呛咳严重程度，使血流动力学更加平稳。

Abstract

Objective

To observe the therapeutic effect of acupuncture at Shuitong and Shuijin points on preventing sufentanil-induced cough and its influence on hemodynamics in general anesthesia induction.

Methods

A total of 80 patients scheduled for elective surgery undergoing general anesthesia were randomly divided into an observation group (40 cases) and a control group (39 cases

1 case eliminated). In the control group

the routine anesthesia was performed

with intravenous injection of 1% sufentanil citrate 0.5 μg/kg

1% propofol (total amount was calculated according to 2 mg/kg) and cisatracurium besilate 0.2 mg/kg. In the observation group

before routine anesthesia induction

acupuncture was applied to Shuitong and Shuijin points on the right and the needles were retained for 30 min. During anesthesia induction

the complications i.e. cough and chest wall stiffness were observed

and the systolic blood

pressure (SBP)

heart rate (HR) and pulse oxygen saturation (SpO

) were monitored 5 min after the patients entered the operation room (T

)

at the moment of intravenous injection of sufentanil (T

) and 2 min after sufentanil injection (T

)

1 min before and after endotracheal intubation (T

) of the two groups

respectively.

Results

During anesthesia induction

the condition of mild

moderate and severe cough in the observation group was superior to that of the control group (

0.05)

the total cases of cough and its total incidence were lower than those of the control group (

0.05). Two cases of chest wall stiffness were present in each group

but without statistical difference between the two groups (

0.05). In comparison of SBP

HR and SpO

at T

and T

the differences were not significant statistically between the two groups (

0.05). SBP and HR increased at T

when compared with those at T

in the control group (

0.05)

but there was no statistical difference in SpO

(

0.05); while

the differences in SBP

HR and SpO

were not significant at T

when compared with those at T

in the observation group (

0.05).

Conclusions

Acupuncture at Shuitong and Shuijin points can effectively prevent from sufentanil-induced cough

reduce the severity of cough and stabilize the hemodynamic indicators.
