摘要

目的：

在体外膈肌起搏（EDP）联合常规针刺的基础上，观察针刺背俞穴对脑卒中气管切开患者的影响。

方法：

将64例脑卒中气管切开患者随机分为试验组（32例，脱落2例）和对照组（32例，脱落2例）。对照组给予EDP联合常规针刺治疗，针刺穴取百会、中脘及双侧风池、曲池、合谷、内关、血海、阴陵泉、三阴交和足三里，每次30 min；试验组在对照组基础上针刺双侧肺俞、脾俞、肾俞，每次30 min。两组针刺均每日1次，每周5 d，共治疗4周。比较两组患者治疗前后肺功能[用力肺活量（FVC）、第1秒用力呼气容积（FEV

）、峰值呼气流速（PEF）、最大吸气压力（MIP）、最大呼气压力（MEP）

]

、膈肌增厚率（DTF）、膈肌移动度（DE）、脑卒中患者姿势控制量表（PASS）评分、Berg平衡量表（BBS）评分，记录两组患者拔管成功率。

结果：

治疗后，两组患者FVC、FEV

、PEF、MIP、MEP均较治疗前提高（

0.001），且试验组高于对照组（

0.01）；两组患者DTF、DE较治疗前提高(

0.001)，试验组DE高于对照组(

0.001)；两组患者PASS、BBS评分较治疗前提高（

0.001），且试验组BBS评分高于对照组（

0.001）。试验组拔管成功率为73.3%（22/30），高于对照组的46.7%（14/30，

结论：

在EDP联合常规针刺的基础上，针刺背俞穴可改善脑卒中气管切开患者的肺功能、呼吸肌肌力、躯干控制和平衡能力，提高拔管成功率。

Abstract

Objective

To observe the effect of acupuncture at back-

shu

points on patients with post-stroke tracheotomy on the basis of extracorporeal diaphragmatic pacing (EDP) combined with conventional acupuncture.

Methods
