摘要

总结白伟杰教授运用“刘氏毫火针疗法”治疗膝关节骨关节炎（KOA）的临证经验与学术特色。白伟杰教授认为，KOA发病以风寒湿邪为标，肝肾虚损为本，其核心病机为筋骨失衡，辨证以经筋理论为主。在治疗上，其特色体现在以下两个方面：一是基于经筋辨证，注重“查灶辨病”，以浮郄、膝阳关、膝关三穴联动调经筋，伏兔、秩边、居髎三穴协同解筋结，配以太溪、昆仑滋阴益肾、强筋壮骨、舒筋活络，并辅以输穴缓解关节疼痛的精准取穴思路；二是采用“刘氏毫火针疗法”，以量化烧针、细针深刺、速刺、顿刺为技术要点，实现燔针劫刺以焠通经络、激发经气、爆动气血，针出邪散。治疗原则力求精简，取穴少而精，特色鲜明，直达病所，为中医针灸治疗KOA提供了特色鲜明的临床路径。
