摘要

目的：

观察通督解郁法针刺联合局部围刺对肝郁气滞型黄褐斑患者面部色斑及抑郁、焦虑情绪的近期及远期疗效，探讨其“斑郁同治”的临床价值。

方法：

34例肝郁气滞型黄褐斑患者采用通督解郁法针刺（穴取百会、四神聪、神庭、至阳、灵台等）联合皮损区围刺治疗，隔日1次，每周3次，连续治疗12周。于治疗前后评价患者黄褐斑面积及严重指数（MASI）、抑郁自评量表（SDS）、焦虑自评量表（SAS）、中医证候评分；于治疗后评定临床疗效；并于治疗结束后3个月进行随访，记录复发率。

结果：

治疗后，患者MASI、SDS、SAS、中医证候评分均较治疗前降低（

0.05），有效率为91.2%（31/34）。治疗后3个月随访，累计

复发率为17.9%（5/28）。

结论：

通督解郁法针刺联合局部围刺能改善肝郁气滞型黄褐斑患者的皮损状况和负性情绪，且具有一定远期疗效。

Abstract

Objective

To observe the short-term and long-term efficacy of

Tongdu Jieyu

(unblocking governor vessel and relieving depression) acupuncture combined with local surrounding needling on facial pigmentation

depression and anxiety in patients with melasma of liver

stagnation

and explore its clinical value in "simultaneously treating spots and depression".

Methods

A total of 34 patients with melasma of liver

stagnation were treated with

Tongdu Jieyu

acupuncture [Baihui (GV20)

Sishencong (EX-HN1)

Shenting (GV24)

Zhiyang (GV9)

and Lingtai (GV10) etc. were selected

]

combined with surrounding needling of the lesion area. The treatment was given once every other day

3 times per week for 12 consecutive weeks. Before and after treatment

the scores of melasma area and severity index (MASI)

self-rating depression scale (SDS)

self-rating anxiety scale (SAS) and TCM syndrome were evaluated

the clinical efficacy was evaluated after treatment

and the recurrence rate was recorded after 3 months of treatment completion.

Results

After treatment

the patientsʹ MASI

SDS

SAS and TCM syndrome scores were reduced compared with those before treatment (

0.05). The clinical effective rate was 91.2% (31/34). After 3 months of treatment completion

the recurrence rate was 17.9% (5/28).

Conclusion

Tongdu Jieyu

acupuncture combined with local surrounding needling can improve both skin lesions and negative emotions in patients with melasma of liver

stagnation，and demonstrates certain long-term efficacy.
