摘要

目的

观察涌泉灸对肾精亏虚型卒中后认知障碍（PSCI）患者认知功能、下肢运动功能的影响。

方法

将84例肾精亏虚型PSCI患者随机分为观察组和对照组，每组42例，各脱落1例。对照组采用药物、电针、康复训练和重复经颅磁刺激（rTMS）治疗，观察组在对照组治疗基础上联合艾灸双侧涌泉穴治疗。两组治疗均每日1次，每周5 d、休息2 d，连续治疗4周。于治疗前后观察两组患者蒙特利尔认知评估量表（MoCA）、简易精神状态检查量表（MMSE）、Fugl-Meyer下肢运动功能评定量表（FMA-LE）、Berg平衡量表（BBS）、功能独立性评定量表（FIM）、修订版跌倒效能量表（MFES）、中医证候积分量表（SDSVD）评分。

结果

治疗后，两组患者MoCA、MMSE、FMA-LE、BBS、FIM、MFES评分均较治疗前提高（

0.05），观察组高于对照组（

0.05）；两组患者SDSVD 评分较治疗前降低（

0.05），观察组低于对照组（

0.05）。

结论

涌泉灸能改善肾精亏虚型PSCI患者认知功能，提高下肢运动及平衡功能，降低跌倒的风险，提升生活质量。

Abstract

Objective

To observe the effects of moxibustion at Yongquan（KI 1） on the cognitive function and lower limb motor function in patients with post-stroke cognitive impairment of kidney essence deficiency.

Methods

Eighty-four patients with post-stroke cognitive impairment of kidney essence deficiency were randomly divided into an observation group（42 cases，1 case dropped off）and a control group（42 cases，1 case dropped off）.The control group was treated with medication，electroacupuncture，rehabilitation training and repetitive transcranial magnetic stimulation（rTMS）；on the basis of the treatment as the control group，moxibustion at bilateral Yongquan（KI 1）was adopted in the observation group.Both groups were treated once a day，5 days a week with 2-day interval，4 weeks were required. Results

After treatment，the MoCA

FIM and MFES scores were higher than those before treatment in both groups (

0.05)

and the scores in the observation group were higher than those in the control group (

0.05). After treatment，the SDSVD scores were lower than those before treatment in both groups (

0.05)

and the SDSVD score in the observation group was lower than that in the control group (

0.05).

Conclusion

Moxibustion at Yongquan（KI 1） can improve the cognitive function and motor and balance function of lower limbs in patients with post-stroke cognitive impairment of kidney essence deficiency，reduce the risk of fall and improve the quality of life.
