摘要

目的

观察任脉及三阴经筋针刀松筋解结术治疗强直性脊柱炎（AS）的临床疗效。

方法

选择30例伴有脊柱活动受限的AS患者，予任脉及三阴经筋针刀松筋解结术治疗，隔3 d治疗1次，每周2次，4次为一疗程，治疗1个疗程。分别于治疗前后观察患者中医证候、Bath强直性脊柱炎疾病活动性指数（BASDAI）、强直性脊柱炎疾病活动性（ASDAS-ESR）、Bath强直性脊柱炎机体功能活动量化指数（BASFI）、Bath强直性脊柱炎健康状况影响综合评定（BAS-G）、Bath强直性脊柱炎运动功能计量指数（BASMI）评分，脊柱活动度、脊柱疼痛视觉模拟量表（VAS）评分、晨僵时间及红细胞沉降率（ESR）、C反应蛋白（CRP）水平。于治疗后评定国际脊柱关节炎协会（ASAS）20、BASDAI50达标情况及临床疗效。

结果

治疗后，患者中医证候评分及BASDAI、ASDAS-ESR、BASFI、BAS-G、BASMI评分较治疗前降低（

0.01）；腰椎屈曲、腰椎侧屈、胸廓活动度较治疗前增加（

0.01），指地距、枕墙距较治疗前减小（

0.01）；脊柱疼痛VAS评分、晨僵时间及ESR、CRP水平较治疗前降低（

0.01）；

ASAS20、BASDAI50应答率分别为53.3%（16/30）和23.3%（7/30），总有效率为96.7%（29/30）。

结论

任脉及三阴经筋针刀松筋解结术可有效治疗伴脊柱功能活动受限的AS，减轻疼痛、改善AS疾病活动及脊柱功能受限。
